Samuel Masket, Nicole Fram, and Gavin Bahadur are skilled ophthalmologists that provide a range of services to help you attain the clearest vision possible. Our services cover a range of eye problems, including cataracts, glaucoma, corneal disease and dry eye, retinal disorders, and ocular injuries. We have the common goal of ascertaining the most appropriate care for any given patient, explaining the purpose of the treatment, and in initiating said therapy in a courteous and compassionate manner, irrespective of age, gender, nationality, race, or payment program.

Patient Information

We take pride in providing exceptional comprehensive care to all patients. In order to do so, we ask that you download the New Patient Forms in the patient information section prior to your appointment and bring them on the day of your exam or you can fax them ahead of time to 310-229-1222. These forms help us better understand your visual concerns, overall physical health, and assess your risk of ocular disease and vision-related problems.

It is often helpful if you have a written list of all systemic and ocular medications prepared ahead of time.

When you're a new patient, you may download: New Patient Forms ahead of time and either bring them with you or FAX them to our office prior to your appointment. Our fax number is: 310-229-1222.

In addition to the forms you'll be filling out, you will be asked questions regarding your health history, vision-related concerns and expectations. Your answers help us understand the health of your eyes, your risk of eye disease and vision problems, and allow us to make the best possible evaluations and suggestions regarding your current situation.

Before your appointment, find out if you will be given drops to dilate your eyes. Although the effects of dilating drops are temporary, you may need to arrange for someone else to drive or to pick you up after your exam. If you are scheduled for a dilated exam, please expect to be in the office for two hours.

Office Hours

Regular office hours are Monday through Friday from 8:00 AM to 5:30 PM.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy, on your part, will make it possible to give your appointment to another patient.

Payment Plans

Most medical insurance plans will not typically cover cosmetic or laser vision correction procedures. Our office is able to assist you with a financing program for elective medical procedures. Our billing office would be pleased to provide you with further details.
