In order to do so, we ask that you download the New Patient Forms prior to your appointment and bring them on the day of your exam or you can fax them ahead of time to 310-229-1222. These forms help us better understand your visual concerns, overall physical health, and assess your risk of ocular disease and vision-related problems.
It is often helpful if you have a written list of all systemic and ocular medications prepared ahead of time.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Most medical insurance plans will not typically cover cosmetic or laser vision correction procedures. Our office is able to assist you with a financing program for elective medical procedures. Our billing office would be pleased to provide you with further details.
